We’re always happy to welcome new patients and families into our care. Becoming part of our clinic means more than just scheduling an appointment — it means building a long-term partnership with a healthcare provider who listens, informs, and supports your health journey.
Getting Started
Your first step is to request a New Patient Packet from our office. This packet includes all the paperwork needed to get started, including health history forms, patient consent documentation, and our office policies. You’ll also find information about billing, insurance, and your rights and responsibilities as a patient.
We encourage you to read the information carefully and complete the forms ahead of time if possible. This helps our team better prepare for your visit and ensures we have everything we need to begin providing care.

Your First Visit
Arriving 15 minutes early is recommended to allow time for check-in and any last-minute paperwork.

What to Bring to Your First Appointment
- Photo ID (such as a driver’s license or state-issued ID)
- Insurance card(s)
- A list of current medications and supplements, including dosages
- Any previous medical records, labs, or imaging results (if available)
- A list of questions or concerns you’d like to address
What to Expect at Your First Visit
Your first visit will be focused on getting to know you. We’ll review your health history, perform a physical exam, and talk about any current concerns or symptoms. If needed, we’ll run basic labs or schedule follow-up tests. Most importantly, we’ll create a care plan that reflects your needs, preferences, and long-term goals.
Insurance and Financial Assistance
Mid-Michigan Health Centers accepts most major insurance plans, including Medicare and Medicaid. If you’re unsure whether your plan is accepted, call us in advance—we’re happy to verify your benefits.
If you’re uninsured or underinsured, ask us about your options. We’ll explain costs in advance, provide receipts for reimbursement, and work with you to keep care affordable.
Patient Education and Ongoing Support
Our job doesn’t end after your appointment. We provide ongoing educational materials, care summaries, and tools that help you stay on top of your health. Whether it’s reminders for screenings, follow-up calls, or answering your questions about medications or labs, we’re here to help guide your journey.
